Payment Assistance

We're here to help.

If you do not have health insurance and are eligible for a government sponsored health program, at Community Hospital of San Bernardino we will work directly with you to help you through the application process. If you qualify, these programs can provide both access to healthcare and other vital social services for you and your family.

If you are uninsured and do not qualify for a government sponsored insurance program, you may be eligible for free or discounted services from Community Hospital of San Bernardino, depending on your income level and circumstances. In addition, interest-free, extended payment plans are available to patients who qualify for payment assistance and who are making good-faith efforts to settle their bills.

To apply for our Payment Assistance Program or for help in enrolling in a government sponsored insurance program please stop by our Admitting/Patient Registration department or schedule an appointment by calling:

Our staff will provide you with an application, which you will need to complete and return with income and resources information. Our staff can also help determine if you are eligible for an insurance program and assist you in completing your application.
